should be above RM 3,000 a month or equivalent
to RM 36,000 per annum for Indonesian Maids. |

requirements by the Immigration Department to
apply for a foreign maid |

Borang J or J Form (Income Tax) the latest and
original - Both Husband and Wife. |

A
photocopy of Identity Card (Front and Back) -
Both Husband and Wife. |

• |
Address Verification if the address on the
application form differs from Identity Card
address submit either one of the following
originals (e.g. Telephone Bill, Water Bill,
Electricity, Astro, Celcom, and etc - Address
under husband's name). |

Photocopy of marriage certificate. |
| • |
Photocopy of children's birth certificate
(Under 15 years of age). |

Salary confirmation by Employer on Company's
Letter Head, strictly original copy (Both
Husband and Wife). |

Latest Salary Slip for both Husband and Wife
each for the last three months. |

DOCUMENTS WILL BE RETURNED TO YOU ONCE YOUR
APPLICATION HAS BEEN APPROVED BY THE
